Ukiah Veterinary Hospital Boarding Agreement

We would appreciate if you would take a few moments to answer the following question. Please be assured that we do not share personal information about you without your permission.

    Emergency contacts

    Check out time is noon. If your pet stays past noon, there will be additional charges. Additionally, there is an extra charge for the administration of medication.
    As a courtesy to our clients, we offer a 9am or 4pm pickup/drop-off time on days the hospital is closed. We do not check messages or return calls when we are closed. All questions and concerns, paperwork and payments must be submitted IN ADVANCE during regular business hours.

    Pets are fed Science Diet twice a day. We will also feed your pet a diet provided by you, please indicate the details in the special instructions section.
    We provide bedding for all pets that are not destructive. Alternatively, you may provide bedding if you label it in advance. We are unable to guarantee its return in original condition, however.

    Dogs receive a complimentary bath after 7 consecutive days of paid boarding. Exceptions: severely matted or aggressive pets.

    For and additional fee, we offer twice daily exercise.

    For Your Pet’s Health:

    Vaccinations – For the protection of all pets under our care, the following vaccination must be up to date and vouched for by a veterinarian.

    Dogs – Rabies, DA2PP (Distemper/Parvo), Bordatella (Kennel Cough), Influenza
    Cats – Rabies, FVRCP (Distemper)
    Medical Illness – One of the advantages of boarding your pet at a veterinary hospital is that the veterinary attention is readily available should the need arise.

    If your pet becomes ill, we will call the emergency number listed above regarding your pet’s symptoms, treatment options, and estimate of additional costs. If no one can be reached, however, please indicate your wishes below should your pet require treatment to relieve immediate discomfort of to resolve an important medical condition.

    I authorize the doctor to perform whatever services he deems necessary for the best care of my pet until someone can be reached. This included only non-elective treatments and any necessary diagnostics.

    Sedation - If it is necessary to sedate you pet for reason of health and safety, or excessive barking, may we do so?

    Note: There is an additional charge to administer a sedative. If you do not authorize sedation, who will pick up your pet should the need arise?

    I authorize the Ukiah Veterinary Hospital to update my pet’s vaccinations in accordance with the above policy. I intend to pick up my pet on the date specified at the time of drop-off. If circumstances change, I will notify the Ukiah Veterinary Hospital of a new pick up date.
